  • Provide Physical Therapist services including assessment, evaluation, and teaching activities
  • Update care plans as needed and submit visit notes as requested
  • Perform OASIS time point assessments and discharge planning
  • Attend staff meetings, team conferences and educational in-services
  • Use the 4Ms and provide Age-Friendly Care Qualifications
    • Current State Physical Therapy License
    • Good driving record, proof of insurance, and reliable transportation
    • One year home care experience preferred
    • Ability to make sound clinical judgment and assess patient needs
